For many people with disabilities, barriers in their communities take away or severely limit their ability to live independently.  The Center’s Community Supports programs provide a holistic approach to support the diverse needs of individuals wanting to gain the skills to live independently, maintain employment and be safe in their community.

Participants in Community Support Services have the opportunity to be assessed by an occupational therapist or a certified rehab counselor . If adaptive equipment needs are identified, these solutions can be provided and participants are given time to learn to use the equipment before progressing to living in a more independent setting.

The Center’s Community Supports for Independence Program is the only one of its kind in the St. Louis area. The services are provided in the natural home and the community on a weekly basis for up to two years. This new and innovative program allows individuals with developmental disabilities the opportunity to gain skills and build their natural supports. The structured service begins with an Independent Living Skills assessment completed by an occupational therapist (OT), a certified rehabilitation counselor (CRC) or a licensed professional counselor (LPC). The information provided during this assessment allows the team to provide the necessary supports to increase the skills in the areas of need. The ultimate goal of this time-limited program is to support the participants with achieving their goal to live independently within two years.

The Transitional In-Home Program serves individuals who have acquired a brain injury or head trauma after the age of 22. The participants have a service coordinator through the Department of Health. This service provides training and practice with activities related to daily living and maintenance of a household within the home and community. Supports assist the clients served with utilizing strategies to acquire and retain functional living skills. Emphasis is on providing various teaching strategies directly to the participant and family to allow a more independent living environment.
